Obtaining Official Personnel Records
All Employees
Wage and Tax Statement (W-2)
Submit a request for a duplicate copy via the Defense Finance and Accounting Service (DFAS), or contact the Leave, Payroll, and Workforce Support Branch located in Building 31, Room 3B435. You may also contact your former Payroll Liaison by selecting the Institute or Center in which you were employed.
eOPF
Review and save all personnel history documents in your eOPF prior to your separation date.
myPay
Get your last Leave and Earnings Statement (LES), your W-2, and review your lump sum annual leave payment from myPay. You will have access to myPay for up to one year after separation. After you separate, you will no longer be able to log in with your PIV card; instead, you will need to use your username and password.
Address Changes
If your address changes when you separate from federal service, please update it immediately in myPay. If you do not, your final W-2 will be sent to the address already on file.
If it has been over 120 days since you left the Federal government
Contact the National Archives Records Administration Civilian Personnel Records Center. Further information can be located on NPRC’s website.
If you have transferred to another Federal agency
Your records transfer with you. You can contact your new agency for any required records.
Record of Leave (SF-1150) will be provided to the gaining agency. For questions, contact the NIH File Room at (301) 496-3361.
